Our care managers work closely with physicians, nurses, social workers, and a wide range of medical and nonmedical professionals. Care managers work to meet complex chronic patient needs. They make provisions for current and future needs of patients. Care managers promote quality care which encourages appropriate use of available resources.

Chronic Care Management
A dedicated care team will be assessing the physical, functional, social, psychological, environmental, learning and financial needs of patients. Identify problems, goals and interventions designed to meet patient’s needs, including prioritized goals that consider the patient/caregivers goals, preferences and desired level of involvement in the case management plan. Create a patient centered care plan including objectives, goals and actions designed to meet patient’s needs. Provide appropriate interventions which demonstrate knowledge of and sensitivity toward cultural diversity and religious, developmental, health literacy, and educational backgrounds of the population served. Our care managers are dedicated to keeping your chronically ill patient healthy and out of the hospital.
